The assessment activities contained below have been designed for the following unit(s) of competency:

TLIF4064             Manage fatigue management policy and procedures

This is a summative assessment process.  For the student to be assessed as competent in each Unit of Competency, all questions and activities need to be satisfactorily completed.

Each assessment activity contained within:

Assessment 1: Project
All parts need to be completed.

This is a summative assessment activity, which means it is an assessment of what you have learnt and used towards the assessment of your overall competency.

When answering the Project Task, you must submit your response in the form of a report to be attached in an Office Word Format. You may also be asked to submit forms from the Attached Appendix. You must download and complete the form then re-attach as part of your assessment. The report must be at least 1000 words not including any Forms.

Assessment 2: Case Study
All parts need to be completed.

This is a summative assessment activity, which means it is an assessment of what you have learnt and used towards the assessment of your overall competency.

Assessment 3:     Short Answer Question
This requires the completion of all comprehensive questions.

This is a summative assessment activity, which means it is an assessment of what you have learnt and used towards the assessment of your overall competency.

Task 3 includes Short Answer 9 questions you must research these and attach your answers to the assessment. These questions are checking your knowledge of the Unit of Qualification. Your answers must be a minimum of 100 – 300 words.

Type of Assessment This is a summative assessment activity, which means it is an assessment of what you have learnt and used towards the assessment of your overall competency.
General There are a number of short answer questions, a case study and a project. You must attempt each one. If you have difficulty with understanding the question or completing the answers then talk to your trainer.

Your trainer is there to help you understand and help you demonstrate your understanding, not to complete the answers for you.

You are required to answer all questions and provide as much detail as you can. These questions are not required to be completed under exam conditions.

Assessment 1: Project

The objective of this section is to demonstrate

Project

Complete both Part A & Part B of this project on National Camper Trailers (NCT), the company you currently work for.

Part A is around the Fatigue Management System used therein. Part B is about the same system but seeking feedback.

To assist you further in this project, please refer to the following National Camper Trailer Documents and research industry related regulations and legislation:

 

You must answer all the questions below in report format.

Part A

You need to complete a report of at least 1000 words. This report is to address the following points.

  1. Describe the current legal requirements, liabilities and responsibilities for effective fatigue management within NCT and how do they identify, interpret and adapt to changes in legislation and regulations related to fatigue management.
  2. Obtain and review the existing fatigue management policies and procedures.
    1. Describe how all operations systems and standard operating procedures are reviewed in terms of their compliance with the organisational fatigue management regulations and policy
    2. How does the company ensure the necessary, changes are made to operation systems and standard operating procedures to ensure that they are compliant?
    3. Describe how NCT, in accordance with the chain of responsibility principles, appropriate discussions are held with relevant personnel in supplier or subcontractor companies in the organisational supply chain to ensure their operational systems and standard operating procedures are compliant with the fatigue management regulations and policy.
  3. Describe the internal risks concerning the potential effects of fatigue that are identified or confirmed and reviewed. Also describe the external risks within the organisational services and/or products supply chain concerning the potential effects of fatigue are identified or confirmed and reviewed in accordance with fatigue management regulations and the related chain of responsibility procedures.
  4. Describe how the organisational fatigue risk management system implementation plan is developed or reviewed and improved within NCT to ensure the compliance of this system.
  5. How are NCT’s fatigue risk management policy and procedures developed, reviewed and improved in conjunction with relevant personnel?
  6. Describe how within NCT Reports from designated personnel on the implementation of the organisational fatigue risk management system implementation plan and related policies and procedures are received and interpreted.
  7. How are accidents and safety incidents in NCT reviewed and/or investigated and analysed to identify the extent to which fatigue might have been a contributing factor, and the information provided in the reports indicating that fatigue management policy and procedures are not being correctly implemented analysed and an appropriate managerial response and related action is initiated?
  8. Describe how the opportunities for improvements to the organisational fatigue risk management system implementation plan and related policies and procedures are identified and appropriate action is taken to make the necessary adjustments

 

 

Part B

In this section of Project 1 you need to seek feedback on the Fatigue Management System.

  1. You need to access the completed Fatigue Management System feedback forms from 10 employees of NCT.
    1. Analise and collate the feedback.
  2. When you have collated your feedback information, prepare a small report on:
    1. Appropriate adjustments that would need to be made to the plan, policies and procedures based on the feedback received.
    2. Facilitate systems that assist employees and external stakeholders to identify their own learning needs about fatigue management.
    3. Include charts and graphs to clearing communicate where appropriate.
  3. Describe how you would request approval for the fatigue risk management system implementation plan and related policies and procedures is obtained in accordance with organisational procedures, based upon the feedback you have received and the report you have developed.
  4. Describe how NCT would instigate a distribution and present to relevant organisational personnel and external stakeholders on implementation of Fatigue risk management system implementation plan and the related policies and procedures.
  5. Describe the systems for auditing the effectiveness and efficacy of organisational fatigue risk management strategies, policies and procedures that your company has in place.

 

 

Assessment 2: Case Study

The objective of this section is to demonstrate

Case Study – Fatigue Breaches.

You are the WHS Manager at National Camper Trailers Pty. Ltd (NCT). The company’s CEO Michael Dundee has asked you investigate the latest 2 incidents that have involved NCT personnel.

Incident 1

The first one was an injury to Shane Thomas on the afternoon shift on the 15th May 2016. It appears that Shane had worked excessive hours for the week and that this may have contributed to his injury incurring a back strain. Shane Thomas due to his accident resulting in Medical Treatment and requiring more than 24 hours off work due to the accident is classified as a Reportable Incident. This means that the incident must be reported to the State Authority within 24 hours. This also could result in the company be issued with a Breach of COR rules due to the reason for the accident. You must complete a Form 537 Incident Notification WorkSafe Victoria

Incident 2

The second involved the salesman James Wilkinson. He had a near miss on the Sturt Hwy outside of Hay NSW when he had a micro sleep and his car left the road. In this incident there was no injury and no damage to the vehicle, but again the issue appears to be fatigue, as he had already been on the road since 3:00am and the incident happened at 4:45pm with still 5:30 hour’s travel left in his day.

You have been supplied with the 2 Accident ~ Incident and Investigation Report Forms.

The task set you is to make a detailed report to CEO or the actions to take to ensure that we have in place measures to ensure that these incidents are not repeated.

  1. Describe how should NCT deal with the identified or reported fatigue management policy breaches are investigated in accordance with organisational procedures and regulatory requirements?
  2. Describe the action to be taken to ensure internal and/or external personnel who may have contributed to any breach of fatigue management policy are provided with appropriate feedback and information to avoid recurrence of the breakdown in planned fatigue management processes.
  3. Where organisational procedures or culture is found to have contributed to a breach in fatigue management policy, what appropriate action should be taken to improve procedures or to address the culture in ways that aim to avoid a recurrence of the breakdown in planned fatigue management processes.
  4. Describe how you would report breaches of fatigue management policy. How they would be prepared and submitted to designated personnel with details of action taken to prevent a recurrence, in accordance with NCT procedures.
  5. Following on from your investigation describe how you would:
    1. Instigate an organisational training system that is planned to provide competency-based on-the-job and off-the-job training and assessment opportunities as detailed in the organisational strategic plan.
    2. As part of the plan above ensure that Team leaders, supervisory and training staff are provided with adequate opportunities to develop the required expertise to contribute to the organisational fatigue management training and assessment activities.
    3. How can the company’s organisational budgets and resource allocation strategies be planned to provide adequate resources for the implementation of the organisational fatigue risk management system?
  6. Describe the system you would put in place to ensure that periodic reviews are:
    1. Undertaken of fatigue management training systems and appropriate improvements are made as required.
    2. Undertaken of budgetary and resource allocation arrangements as they relate to implementing the organisational fatigue risk management systems and appropriate improvements are made as required
